CarePlus TLC is seeking a dedicated, compassionate, CNA/Home Health Aid with a passion for serving others to join our team!

This role requires a commitment to delivering high-quality care while maintaining a clean and safe environment for clients. The successful candidate is self-motivated, reliable, compassionate, and committed to creating a positive experience in our client’s lives

Our staff create a friendly, caring environment to ensure our clients receive the quality care and assistance they need to maintain their independence and quality of life in their home.

Jenison Client: 10a-5pm 2-3 days per week. $22/hour.

Responsibilities:

• Knowledge and experience of dementia care is essential
• Monitor patient’s condition and report changes/concerns to appropriate medical professionals and family members
• Provide Activities of Daily Living care as outlined in the Care Plan-bathing, dressing, feeding, grooming, etc.
• Assist client in transfers in a safe and secure manor.
• Perform light housekeeping duties to maintain a safe and clean-living space for clients.
• Provide companionship and emotional support to enhance patient well-being.
• Ability to turn and reposition patients
• Understands First Aid and Emergency Care protocols
• Complete electronic documentation while on shift
• Medication reminders
• Adhere to HIPAA regulations and maintain patient confidentiality.
• Help with mobility and transportation needs for appointments.

Skills:

• Ability to work effectively with individuals with developmental disabilities or dementia is desirable.
• Familiarity with Home Health Care, safety and sanitation standards and procedures
• Understanding of Confidentiality and Privacy Laws
• Excellent communication and interpersonal skills… able to communicate effectively with patients and their families.
• Knowledge of basic medical terminology
• Detailed oriented
• Ability to turn and reposition patients
• Ability to undertake a variety of diverse care tasks and work as a team member
• Compassionate and able to handle tough situations as needed
• 1 year experience as a caregiver is preferred.
