Summary

Summary:

The Health Coach plays a vital role in empowering patients to manage their chronic conditions and achieve theirhealth goals. The health coach will help patients navigate their health journey by providing one-on-one,telephonic health coaching, assessing their lifestyle habits, and supporting them through behavioral changes.Utilizing cutting-edge remote tools and technology to track progress and outcomes, the health coach will offerguidance and education on disease management, medication adherence, and prevention strategies, as well asassist in coordination of care across clinical and community services.The Health Coach role provides individualized wellness coaching and support to patients identified for care management. The Health Coach will partner with patients to ensure improved access to, and coordination of clinical and community services across the continuum of care.

Required Minimum Knowledge, Skills, And Abilities (KSAs)

  • Education: Bachelor’s Degree preferred.
  • License Certification: Valid Driver’s License required. Health Coach Certification preferred.
  • Experience: Two years of experience in a social setting or health care setting or equivalent education preferred.
  • Additional Skills/Requirements Required: N/A
  • Additional Skills/Requirements Preferred: Bilingual. Local community resident with knowledge of resources available in the community. Proficient computer use. Knowledge of Medical terminology.
